Phase 1 (Weeks 0-6): Protection. Your shoulder will be in a sling for 4-6 weeks after surgery. During this phase, your NeoLife therapist will focus on passive range of motion — gently moving your arm without you using any muscle effort. We'll also manage swelling, protect the surgical repair, and begin gentle exercises for your elbow, wrist, and hand to prevent stiffness.
Phase 2 (Weeks 6-12): Active Motion. Once your surgeon clears you, we begin active-assisted and active range of motion exercises. Your therapist will use manual therapy techniques to address any scar tissue adhesions and restore normal shoulder mechanics. Phase 3 (Months 3-5): Strengthening. We progressively load the repaired rotator cuff with resistance exercises, beginning with isometrics and advancing to dynamic strengthening. We'll also address scapular stabilization — the shoulder blade muscles that are critical for healthy shoulder function.
Phase 4 (Months 5-6+): Return to Activity. For athletes and active individuals, we design sport-specific or activity-specific programs to ensure you return to your activities with confidence. We use objective strength testing to confirm the repaired shoulder matches the other side before clearing you.
Recovery from rotator cuff surgery typically takes 4-6 months, though some patients require up to a year for full recovery depending on the size of the tear and the type of repair. The quality of your physical therapy during this time directly impacts your long-term outcome.
