
Total knee replacement — also called total knee arthroplasty — is one of the most successful surgeries in all of medicine. Over 1 million knee replacements are performed in the US each year, and physical therapy is the single most important factor in determining how well you recover. Before Surgery: Prehabilitation. Research shows that patients who participate in physical therapy before surgery recover faster and have better outcomes after surgery. At NeoLife, our prehab program focuses on maximizing your knee range of motion, strengthening your quadriceps and hip muscles, improving your overall fitness, and teaching you the exercises you'll need immediately after surgery.
Week 1: Hospital and Early Home Recovery. Physical therapy begins the day of or day after your surgery. The initial focus is on knee bending and straightening exercises, getting in and out of bed safely, walking with your assistive device, and managing swelling with ice and elevation. Most patients are discharged home within 1-2 days.
Weeks 2-6: Outpatient Physical Therapy. This is when you'll begin outpatient PT at NeoLife. Sessions typically occur 2-3 times per week. Your therapist will use manual therapy to progressively restore knee range of motion — the goal is 0 degrees of extension (fully straight) and at least 120 degrees of flexion (bending). We'll also begin progressive strengthening, gait training to normalize your walking pattern, stair training, and scar tissue mobilization.
Weeks 6-12: Progressive Strengthening. By now, most patients are walking without an assistive device and have good range of motion. We shift focus to building functional strength, improving balance and proprioception, returning to household activities and recreational walking, and addressing any lingering stiffness or swelling with manual techniques.
Months 3-6: Return to Full Activity. The final phase focuses on advanced strengthening, sport-specific or activity-specific training (golf, gardening, tennis, etc.), endurance building, and confidence in the new knee. Most patients are back to their normal activities by 3-4 months, with continued improvement through 6 months and beyond.
